Overview

A combination therapy consisting of lopinavir, ritonavir, and hydroxychloroquine that was investigated for the treatment of COVID-19, particularly in the early stages of the pandemic. This triple therapy combines two different mechanisms of action: - **Lopinavir/ritonavir**: Lopinavir is an HIV-1 protease inhibitor that was hypothesized to inhibit the SARS-CoV-2 main protease (3CLpro or Mpro). Ritonavir is co-administered to boost lopinavir levels by inhibiting CYP3A4 metabolism. - **Hydroxychloroquine**: An autophagy inhibitor that alkalinizes phagolysosomes, potentially impairing viral uncoating and cellular fusion. It may also have immunomodulatory effects by decreasing the production of proinflammatory cytokines. Clinical trials, including the WHO Solidarity Trial, found that this combination produced little or no reduction in mortality for hospitalized COVID-19 patients compared to standard care.
