This pharmacological regimen is a combination therapy consisting of the angiotensin II receptor blocker (ARB) losartan, the calcium channel blocker (CCB) amlodipine, and the HMG-CoA reductase inhibitor (statin) atorvastatin. Developed for use in the rrAD (Exercise and Intensive Vascular Risk Reduction in Preventing Dementia) clinical trial led by the University of Texas Southwestern Medical Center, the regimen is designed to intensively manage vascular risk factors—specifically hypertension and hyperlipidemia—to evaluate its impact on cognitive performance and the prevention of Alzheimer's disease and vascular dementia in high-risk older adults. Losartan acts by antagonizing the AT1 receptor, amlodipine inhibits L-type calcium channels to promote vasodilation, and atorvastatin reduces cholesterol synthesis by inhibiting the HMG-CoA reductase enzyme. The combination aims to achieve a systolic blood pressure target of less than 130 mmHg and significant lipid reduction as part of a multi-modal approach to neuroprotection.
