Overview

LD + I + TRB is a combination intervention developed for use during and after transradial coronary angiography to prevent radial artery occlusion (RAO). It consists of a low-dose systemic administration of heparin (typically 2000-3000 IU), the application of the InnoSEAL hemostatic pad, and the use of a TR Band for compression. Heparin serves as an anticoagulant by binding to antithrombin III, which then inactivates thrombin and factor Xa, thereby inhibiting the coagulation cascade. The InnoSEAL pad is a catecholamine-functionalized chitosan-based device that promotes rapid hemostasis through biomimetic adhesion at the arterial puncture site. The TR Band is a transparent radial compression device that allows for patent hemostasis by applying pressure while maintaining blood flow. This combination approach, studied by the Tabba Heart Institute in the InnoSEAL III trial, aims to minimize hemostasis time and reduce the incidence of RAO and hematomas compared to standard-dose heparin or compression alone.
