Overview

The combination of low molecular weight heparin (LMWH) and insulin is a therapeutic regimen primarily utilized for the management of acute hypertriglyceridemic pancreatitis (HTGP). This treatment strategy, notably developed and evaluated by The First Affiliated Hospital of Nanchang University, leverages the synergistic effects of both agents to rapidly reduce serum triglyceride levels. Insulin acts by stimulating the synthesis and activation of lipoprotein lipase (LPL), an enzyme that catalyzes the hydrolysis of triglycerides within chylomicrons and very-low-density lipoproteins. LMWH further enhances this process by promoting the release of LPL from the vascular endothelial surface into the systemic circulation. Beyond its metabolic effects, LMWH provides essential anticoagulation to prevent microcirculatory thrombosis, a frequent complication in severe pancreatitis. This combination serves as a non-invasive, accessible, and cost-effective alternative to therapeutic plasma exchange (plasmapheresis) in clinical settings.
