Overview

Low-molecular-weight heparins + direct oral anticoagulants is a non-standardized combination anticoagulant regimen in which a parenteral low-molecular-weight heparin (such as enoxaparin, dalteparin, or tinzaparin) is used together with, or transitioned to, an oral direct factor Xa or direct thrombin inhibitor (such as apixaban, rivaroxaban, edoxaban, or dabigatran). Low-molecular-weight heparins are glycosaminoglycan fragments that potentiate antithrombin-mediated inhibition of factor Xa (and to a lesser extent thrombin), while direct oral anticoagulants selectively and directly inhibit factor Xa or thrombin without a cofactor, producing systemic anticoagulation for prevention and treatment of venous thromboembolism. Concurrent co-administration of LMWH with DOACs is generally avoided because of an increased bleeding risk, but short overlapping or sequential use is common during transitions of therapy in clinical practice and trials evaluating cancer-associated thrombosis and other high-risk thrombotic states.[1][2][3]
