**Low-titer group O+ non-leukoreduced whole blood (LTOWB)** is a complete blood product collected from group O RhD-positive donors with low levels of anti-A and anti-B antibodies (typically titers <256, varying by blood center). It contains red blood cells, cold-stored platelets, and plasma in a single unit, stored cold for up to 21 days, and is non-leukocyte-reduced. Primarily indicated for rapid, balanced resuscitation in life-threatening hemorrhage, such as trauma, where oxygen-carrying capacity, coagulation factors, platelets, and volume expansion are needed urgently, especially when patient blood type is unknown. It serves as a universal donor product, simplifying logistics over separate components, reducing donor exposures, and minimizing dilutional coagulopathy risks.[1][2][3]
