LY2605541 + insulin lispro

LY2605541 + insulin lispro is an investigational combination therapy for diabetes mellitus, specifically studied in type 1 diabetes. LY2605541, also known as basal insulin peglispro (BIL), is a PEGylated form of insulin lispro, a rapid-acting insulin analog. PEGylation increases the hydrodynamic size, delays absorption, reduces renal clearance, and prolongs the half-life, resulting in a flat and protracted basal insulin profile[1][2]. When combined with prandial (meal-time) insulin lispro, this regimen aims to mimic physiological insulin replacement consisting of a long-acting basal insulin with hepato-preferential action and rapid-acting bolus insulin. Mechanistically, both components are agonists of the insulin receptor, promoting glucose uptake and normalization in patients. Clinical studies indicated improved glycemic control (lower HbA1c), reduced nocturnal hypoglycemia, increased total hypoglycemia, potential for elevated liver enzyme levels and triglycerides, and increased risk of lipohypertrophy at injection sites. Development of LY2605541 as a basal insulin was discontinued due to concerns about liver fat accumulation and hepatic safety[1][2].
