LY3039478 + dexamethasone is a **combination therapy** composed of LY3039478, an orally available small molecule Notch pathway inhibitor, and dexamethasone, a synthetic glucocorticoid corticosteroid. LY3039478 (also known as Crenigacestat) inhibits the proteolytic cleavage of the Notch receptor, blocking Notch signaling—an oncogenic pathway in several malignancies, including T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma (T-LBL). Dexamethasone is added both for its cytotoxic anti-leukemic activity and for its protective effect against gastrointestinal toxicity induced by Notch inhibition. The combination has been studied in Phase 1b/2 clinical trials in adult and pediatric patients with relapsed or refractory T-ALL/T-LBL[1][3][5].
