LY3039478 + taladegib is an investigational combination therapy consisting of LY3039478 (a selective inhibitor of the Notch signaling pathway) and taladegib (also known as LY2940680, a Hedgehog/Smoothened [Smo] pathway inhibitor). LY3039478 acts by inhibiting the Notch receptor, which disrupts Notch pathway signaling frequently implicated in tumorigenesis and cancer stem cell maintenance. Taladegib targets the Smoothened receptor to block Hedgehog pathway signaling, another key driver of cancer cell proliferation and survival. The rationale for combining these agents is based on the preclinical and early clinical evidence that simultaneous inhibition of the Notch and Hedgehog pathways can help target cancer stem cell populations and overcome resistance mechanisms in various malignancies. The combination has been tested in a Phase 1b clinical trial for safety, pharmacokinetic, and preliminary efficacy evaluation in advanced/metastatic solid tumors. In these studies, LY3039478 is administered orally three times weekly and taladegib is administered orally once daily in 28-day cycles[1][3][5].
