Magnesium + artificial cerebrospinal fluid (Mg-aCSF) is a neuroprotective irrigation solution designed to mimic the physiological and electrolyte balance of natural cerebrospinal fluid while providing therapeutic concentrations of magnesium ions. Primarily utilized in neurosurgical procedures, particularly following subarachnoid hemorrhage (SAH), the solution is administered via intracisternal or intraventricular irrigation. Its primary mechanism of action is mediated by magnesium's role as a voltage-dependent, non-competitive antagonist of the N-methyl-D-aspartate (NMDA) receptor, which prevents calcium-induced excitotoxicity in neurons. Additionally, magnesium acts as a natural calcium channel blocker in vascular smooth muscle, promoting vasodilation of cerebral arteries to prevent or mitigate delayed cerebral ischemia and vasospasm. The balanced electrolyte environment of the artificial cerebrospinal fluid carrier helps maintain osmotic and ionic stability in the central nervous system during therapy.
