Magnesium + dexmedetomidine is a combination therapy approach investigated by researchers at Istanbul University for the management of hemodynamic instability during laparoscopic pheochromocytoma excision. The intervention involves pre-operative oral magnesium followed by intra-operative infusions of both magnesium and dexmedetomidine. Magnesium acts as a calcium antagonist, inhibiting the release of catecholamines from the adrenal medulla and nerve endings, and also functions as an alpha-adrenoreceptor antagonist. Dexmedetomidine is a highly selective alpha-2 adrenergic agonist that provides sympatholytic, sedative, and analgesic effects. This combination is designed to reduce the requirement for vasodilators like sodium nitroprusside and to stabilize blood pressure and heart rate during the surgical manipulation of catecholamine-secreting tumors.
