MagSense HER2 Imaging Agent (MSH2IA) is a molecularly targeted magnetic resonance imaging (MRI) contrast agent developed for the non-invasive detection and staging of nodal metastasis in HER2-positive breast cancer. The agent consists of superparamagnetic iron oxide nanoparticles (SPIONs) coated with PEGylated biocompatible polymers and covalently conjugated to the anti-HER2 monoclonal antibody trastuzumab. Following subcutaneous (subareolar or peritumoral) injection, MSH2IA drains into the axillary lymph nodes, where the trastuzumab moiety selectively binds to HER2-expressing metastatic breast cancer cells. The bound nanoparticles alter local magnetic fields, creating a distinct magnetic signature detectable by MRI or superparamagnetic relaxometry (SPMR), thereby allowing clinicians to differentiate metastatic lymph nodes from healthy or normally reactive ones without the need for invasive biopsies.
