Malate acetated Ringer's solution is a balanced crystalloid intravenous fluid used for the replacement of extracellular fluid and the correction of metabolic acidosis. Developed primarily by Otsuka Pharmaceutical (marketed as Physio140), it contains a physiological balance of electrolytes including sodium, potassium, magnesium, calcium, and chloride. Unlike traditional Lactated Ringer's, it uses acetate and malate as buffering agents; these are metabolized into bicarbonate in various tissues (including muscle and liver), providing a more rapid buffering effect and reducing the metabolic burden on the liver. It is indicated for fluid resuscitation in patients with trauma, surgery, or severe dehydration where acid-base balance must be maintained.
