Marizomib + dexamethasone is a combination regimen comprised of marizomib, an irreversible, pan-subunit proteasome inhibitor, and dexamethasone, a synthetic glucocorticoid. Marizomib inhibits all three proteolytically active subunits of the proteasome (chymotrypsin-like, trypsin-like, and caspase-like), leading to disruption of protein metabolism and apoptosis in cancer cells, and is particularly distinct in its ability to cross the blood-brain barrier. Dexamethasone, as a corticosteroid, induces apoptosis and reduces inflammation. The combination is under investigation primarily for relapsed or refractory multiple myeloma, typically as part of a multidrug regimen also including immunomodulatory agents such as pomalidomide.
