**MBS2320 + methotrexate** is a combination therapy currently under investigation for the treatment of **rheumatoid arthritis (RA)**. MBS2320 is described as a first-in-class **metabolic reprogramming agent** that targets both inflammation and bone degradation, with a dual mode-of-action: it selectively modulates immune cell function (inhibiting myeloid and lymphoid activity/differentiation while sparing mesenchymal cells), reduces inflammation, and supports remodelling of damaged tissue and bone protection. Methotrexate is a longstanding cornerstone therapy for RA, acting as an antimetabolite inhibiting folate metabolism, thus suppressing immune activity and inflammation. This combination has been tested in Phase 1, Phase 2a, and Phase 2b clinical trials in patients with active RA, specifically those who have not responded sufficiently to methotrexate alone. The combination does not show pharmacokinetic interactions, and MBS2320 was generally well tolerated alongside background methotrexate therapy, with nausea being the most common mild adverse event.
