mBTC vax + poly-ICLC is a combination immunotherapy consisting of a personalized mutant peptide-based cancer vaccine (mBTC vax) administered together with the immune adjuvant poly-ICLC. The mBTC vax component is a peptide vaccine designed to target tumor-specific neoantigens derived from patient-specific mutations. Poly-ICLC (Hiltonol) is a synthetic double-stranded RNA that acts as an agonist for Toll-like receptor 3 (TLR3) and melanoma differentiation-associated gene 5 (MDA5), mimicking viral infection to stimulate innate immunity. As an adjuvant, poly-ICLC enhances dendritic cell maturation and promotes robust CD4+ and CD8+ T cell responses against tumor antigens presented by the vaccine[3][4][6]. This combination has been investigated in early-phase clinical trials for various cancers including glioblastoma, pancreatic cancer, and melanoma[3][5][7]. The primary mechanism involves induction of antigen-specific cytotoxic T lymphocytes through enhanced antigen presentation and pro-inflammatory cytokine production.
