MCT/LCT-based 20% lipid emulsion (brand name: Lipofundin MCT/LCT 20%) is an intravenous lipid emulsion developed and manufactured by B. Braun Melsungen AG for use in total parenteral nutrition (TPN). It consists of a 1:1 physical mixture of medium-chain triglycerides (MCTs, typically derived from coconut oil) and long-chain triglycerides (LCTs, derived from soybean oil). This formulation provides a concentrated source of energy and essential fatty acids for patients who cannot receive enteral nutrition, particularly critically ill patients in the intensive care unit (ICU). MCTs are more rapidly hydrolyzed and oxidized than LCTs, providing efficient energy utilization and potentially improving nitrogen balance. The emulsion has been evaluated in clinical trials, such as the SEMICYUC phase 4 trial, for its impact on nosocomial infection rates and clinical outcomes in ICU patients compared to LCT-only emulsions.
