Overview

MEL-10 + CFZ-TD-PACE is an intensive eight-drug combination chemotherapy regimen developed by the University of Arkansas for Medical Sciences (UAMS) for the induction phase of the Total Therapy 5b (TT5b) protocol. It is specifically designed for patients with high-risk multiple myeloma, identified by gene expression profiling (GEP70 risk score ≥ 0.66). The regimen consists of a dose-reduced melphalan (10 mg/m²) combined with carfilzomib (a second-generation proteasome inhibitor), thalidomide (an immunomodulatory agent), dexamethasone (a corticosteroid), and the PACE backbone, which includes cisplatin, doxorubicin (Adriamycin), cyclophosphamide, and etoposide. This multi-pathway approach aims to achieve rapid and sustained complete remission by simultaneously targeting DNA replication, protein degradation, and the bone marrow microenvironment to overcome treatment resistance in aggressive disease states.
