MEL-20 + CFZ-TD-PACE is a multi-drug combination chemotherapy regimen developed by the University of Arkansas for the treatment of high-risk multiple myeloma. It is a key component of the Total Therapy 5b (TT5B) protocol, specifically utilized as a non-transplant supported inter-therapy cycle between tandem autologous stem cell transplants. The regimen consists of a low-dose melphalan (20 mg/m²) backbone combined with the proteasome inhibitor carfilzomib, the immunomodulatory drug thalidomide, the corticosteroid dexamethasone, and the PACE chemotherapy backbone (cisplatin, doxorubicin, cyclophosphamide, and etoposide). This intensive approach aims to achieve and sustain complete remission in patients with high-risk cytogenetics by targeting malignant plasma cells through multiple synergistic mechanisms, including DNA alkylation, proteasome inhibition, and modulation of the bone marrow microenvironment.
