Melarsoprol + nifurtimox is a combination therapy used for the treatment of second-stage human African trypanosomiasis (sleeping sickness), particularly caused by Trypanosoma brucei gambiense. Melarsoprol is an arsenic-containing small molecule that acts as a trypanocidal agent by irreversibly binding to sulfhydryl groups on the enzyme pyruvate kinase, disrupting energy production in the parasite and inhibiting trypanothione reductase. Nifurtimox is a nitrofuran derivative that generates reactive oxygen species, leading to oxidative stress and damage in the parasite. The combination regimen typically involves intravenous melarsoprol with oral nifurtimox over 10 days. Clinical studies have shown this combination to be more effective than standard melarsoprol monotherapy, with similar rates of adverse events[1][4]. This regimen was developed as an alternative due to increasing resistance and toxicity concerns associated with melarsoprol alone.
