Melatonin + magnesium sulfate is an investigational combination therapy being evaluated for its neuroprotective effects in term neonates with hypoxic-ischemic encephalopathy (HIE). Melatonin, an endogenous hormone, acts as a potent antioxidant and free radical scavenger, while magnesium sulfate serves as an inorganic salt that functions as a non-competitive NMDA receptor antagonist. Together, they aim to mitigate brain injury by reducing oxidative stress and excitotoxicity. This combination is currently being studied in Phase 3 clinical trials sponsored by the University of Child Health Sciences and Children's Hospital, Lahore, comparing the dual therapy against magnesium sulfate monotherapy.
