Overview

**melphalan + panobinostat** is an investigational combination of two small molecule drugs for oncology indications, particularly multiple myeloma. Melphalan is an alkylating agent that induces cytotoxicity via DNA interstrand crosslinking, leading to DNA damage and cell death. Panobinostat is a pan-histone deacetylase (HDAC) inhibitor that alters chromatin structure and downregulates DNA repair enzymes, thereby enhancing DNA damage and apoptosis induced by genotoxic agents such as melphalan. Together, this combination demonstrates synergistic effects in multiple myeloma cells by (1) inhibiting the repair of melphalan-induced DNA adducts and double-strand breaks, (2) increasing accumulation of DNA lesions, and (3) promoting apoptosis selectively in malignant plasma cells while sparing non-malignant blood cells. This selectivity suggests potential for improved efficacy and reduced side effects compared to melphalan alone[1][2][3].
