Overview
Melphalan + prednisolone is an oral combination chemotherapy regimen primarily used as first-line or relapsed therapy for multiple myeloma in patients who are not eligible for stem cell transplantation. Melphalan is a bischloroethylamine alkylating agent that cross-links DNA at the N7 position of guanine, disrupting DNA synthesis and transcription, leading to cytotoxicity in both resting and dividing tumor cells. Prednisolone is a synthetic glucocorticoid with anti-inflammatory and immunosuppressive properties, which also induces apoptosis in certain hematologic malignancies. The combination exploits the cytotoxic effects of melphalan with the lympholytic and anti-myeloma activity of prednisolone[1][2][5].
