Overview

Memantine hydrochloride extended-release + donepezil hydrochloride (marketed under the brand name Namzaric) is a once-daily, fixed-dose combination capsule approved for the treatment of moderate to severe dementia of the Alzheimer's type. It combines two established therapeutic agents with complementary mechanisms of action: memantine hydrochloride, an uncompetitive, moderate-affinity N-methyl-D-aspartate (NMDA) receptor antagonist, and donepezil hydrochloride, a reversible acetylcholinesterase (AChE) inhibitor. Memantine works by binding preferentially to NMDA receptor-operated cation channels to block the tonic pathological activation of glutamate, thereby protecting neurons from excitotoxicity without interfering with physiological transmission. Donepezil enhances cholinergic function by reversibly inhibiting the acetylcholinesterase enzyme, which increases the concentration of acetylcholine available for synaptic transmission in the central nervous system. Originally developed by Adamas Pharmaceuticals and Forest Laboratories (later Allergan/AbbVie), this combination is designed to simplify the dosing regimen and improve compliance in patients stabilized on donepezil. Generic versions have been launched by companies such as Amneal Pharmaceuticals, and bioequivalence studies are being conducted by regional developers like Sichuan Credit Pharmaceutical in China.
