Overview

Mepolizumab + omalizumab is a non-fixed, off-label combination of two humanized monoclonal antibodies used together in select cases of severe asthma and allergic bronchopulmonary aspergillosis. Mepolizumab targets interleukin-5 (IL-5), reducing eosinophil levels and activity, while omalizumab binds to immunoglobulin E (IgE), preventing its interaction with IgE receptors on mast cells and basophils. This dual approach aims to address both the eosinophilic and allergic components of severe asthma or related conditions that are refractory to single-agent biologic therapy. The combination has been reported in case studies for patients with overlapping phenotypes—such as high IgE levels and persistent eosinophilia—who do not respond adequately to either agent alone[1][2][4]. There is no approved fixed-dose product; use is based on clinical judgment in highly selected cases.
