Meropenem + sargramostim + human albumin is a combination therapeutic regimen investigated by the Institute of Liver and Biliary Sciences (ILBS) in India for the treatment of difficult-to-treat spontaneous bacterial peritonitis (SBP) in patients with decompensated cirrhosis. The regimen combines meropenem, a broad-spectrum carbapenem antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), with sargramostim (granulocyte-macrophage colony-stimulating factor or GM-CSF), a cytokine that stimulates the production and activation of neutrophils and macrophages to overcome the immune dysfunction typically seen in advanced cirrhosis. Human albumin is included to provide oncotic support and reduce the incidence of renal impairment. Clinical trial results indicated that the addition of GM-CSF to the standard carbapenem and albumin therapy significantly improved response rates in patients with treatment-resistant SBP.
