Overview

**meropenem + tobramycin** is a combination of two antibiotics, **meropenem**, a broad-spectrum carbapenem β-lactam, and **tobramycin**, an aminoglycoside. This combination is primarily researched for the treatment of severe infections caused by multidrug-resistant *Pseudomonas aeruginosa*, particularly in cystic fibrosis and pneumonia. Meropenem acts by inhibiting cell wall synthesis via binding to penicillin-binding proteins, while tobramycin inhibits protein synthesis by binding to the bacterial 30S ribosomal subunit and also disrupts the outer bacterial membrane. The combination exhibits **synergistic activity and enhanced bacterial killing** compared to monotherapies, suppressing resistance amplification especially when dosing is optimized (e.g., continuous infusion of meropenem with standard tobramycin dosing). Research shows additive or synergistic interactions, effective suppression of regrowth and resistance in both planktonic and biofilm bacteria[1][2][3].
