This combination treatment regimen consists of the anti-inflammatory agent 5-aminosalicylic acid (mesalamine) and the systemic corticosteroid methylprednisolone. It was primarily investigated in a Phase 4 randomized controlled clinical trial (NCT01452412) led by Dr. Shomron Ben-Horin at Sheba Medical Center to determine if adding or continuing 5-ASA therapy provides additional benefit when combined with standard intravenous corticosteroids in patients hospitalized for moderate-to-severe acute exacerbations of ulcerative colitis. Mesalamine acts locally on the colonic mucosa to inhibit inflammatory mediators such as prostaglandins and leukotrienes by modulating PPAR-gamma and NF-κB pathways. Methylprednisolone provides potent systemic anti-inflammatory and immunosuppressive effects by acting as an agonist at the glucocorticoid receptor. The study concluded that the addition of 5-ASA to corticosteroids did not significantly improve short-term clinical outcomes compared to corticosteroids alone in this acute setting.
