mesalamine + methylprednisolone + hydrocortisone is a combination therapy evaluated by Sheba Medical Center for the treatment of moderate-to-severe active ulcerative colitis. This regimen combines mesalamine (5-aminosalicylic acid or 5-ASA), a locally acting anti-inflammatory agent, with systemic corticosteroids such as methylprednisolone or hydrocortisone. Mesalamine is thought to exert its effects by activating peroxisome proliferator-activated receptor gamma (PPAR-gamma) and inhibiting cyclooxygenase and lipoxygenase pathways in the colonic mucosa. Corticosteroids provide broad immunosuppression by acting as agonists at the glucocorticoid receptor, reducing the production of pro-inflammatory cytokines. This specific combination was the subject of a Phase 4 randomized controlled trial (NCT01941589) to determine if the addition of 5-ASA to standard corticosteroid therapy provides superior efficacy in patients experiencing acute exacerbations of ulcerative colitis.
