Overview

Mesalamine + n-acetylcysteine is an oral combination therapy investigated for the treatment of inflammatory bowel diseases, particularly ulcerative colitis. Mesalamine (also known as mesalazine or 5-aminosalicylic acid) is an anti-inflammatory aminosalicylate that acts primarily in the colon to reduce inflammation by inhibiting cyclooxygenase and lipoxygenase pathways, thereby decreasing prostaglandin and leukotriene synthesis. N-acetylcysteine (NAC) is a precursor to glutathione and functions as an antioxidant by scavenging free radicals and reducing oxidative stress. The rationale for combining these agents is that NAC may enhance the anti-inflammatory effects of mesalamine by mitigating oxidative tissue damage associated with ulcerative colitis. Clinical studies have shown that this combination can improve clinical remission rates and decrease pro-inflammatory chemokines such as MCP-1 and IL-8 without increasing adverse events compared to mesalamine alone[1][3][4].
