A multi-component therapeutic regimen developed by the Mario Negri Institute for Pharmacological Research for use in kidney transplantation. The protocol involves the intravenous infusion of syngeneic, ex-vivo expanded mesenchymal stem cells (MSCs) to induce immunomodulatory effects. This cell therapy is combined with an induction regimen consisting of basiliximab (a monoclonal antibody targeting the IL-2 receptor), low-dose rabbit anti-thymocyte globulin (RATG) for T-cell depletion, and methylprednisolone. Maintenance immunosuppression is provided via low-dose cyclosporine (a calcineurin inhibitor) and mycophenolate mofetil (an antimetabolite). The primary objective of this combination is to establish a pro-tolerogenic environment that may eventually allow for the safe withdrawal of maintenance immunosuppressive drugs in living-related kidney transplant recipients.
