Mesenchymal stromal cells, ex vivo cultured (specifically the product Obnitix developed by Medac), is an allogeneic cell therapy consisting of bone marrow-derived mesenchymal stromal cells (MSCs) that have been expanded in vitro. It is primarily indicated for the treatment of steroid-refractory acute graft-versus-host disease (aGvHD). The therapeutic effect is mediated through the immunomodulatory properties of MSCs, which involve the secretion of various anti-inflammatory factors such as indoleamine 2,3-dioxygenase (IDO), prostaglandin E2 (PGE2), and transforming growth factor-beta (TGF-β). These factors collectively suppress the activation and proliferation of alloreactive T cells and other immune effectors, while promoting the induction of regulatory T cells (Tregs), thereby reducing the systemic inflammatory response and tissue damage associated with GvHD.
