Aldaid is a fixed-dose combination hepatoprotective medication containing metadoxine (500 mg) and silymarin (140 mg), developed and marketed by Delvin Formulations. It is indicated for the treatment of various liver disorders, including alcoholic liver disease, non-alcoholic fatty liver disease (NAFLD), liver cirrhosis, and drug-induced liver injury. Metadoxine, an ion-pair salt of pyridoxine and pyrrolidone carboxylate, acts as a selective 5-HT2B receptor antagonist and GABA modulator; it accelerates the metabolism and clearance of ethanol and acetaldehyde by maintaining alcohol dehydrogenase (ADH) and acetaldehyde dehydrogenase (ALDH) levels, thereby reducing alcohol-induced oxidative stress and hepatotoxicity. Silymarin, a natural flavonolignan complex extracted from milk thistle (*Silybum marianum*), serves as a potent antioxidant and free-radical scavenger that stabilizes hepatocyte membranes, prevents lipid peroxidation, and promotes liver cell regeneration. Together, the components of Aldaid synergistically protect the liver from toxic insults, reduce inflammation, and support hepatic function.
