Overview

Metformin + cyclosporine is a combination therapy being investigated for the treatment of moderate to severe psoriasis. Metformin, a biguanide primarily used for type 2 diabetes, is being studied for its anti-inflammatory and immunomodulatory properties, specifically its ability to activate AMP-activated protein kinase (AMPK) and inhibit the mTOR pathway, which are relevant in psoriatic pathogenesis. Cyclosporine is a potent immunosuppressant that inhibits calcineurin, thereby reducing T-cell activation and cytokine production. This combination aims to enhance the clinical efficacy of psoriasis treatment while potentially allowing for lower doses of cyclosporine, thereby mitigating its long-term adverse effects. The regimen is currently being evaluated in comparative clinical trials sponsored by Cairo University.
