Metformin + digital lifestyle intervention is a combined pharmacological and behavioral approach being evaluated by St. Jude Children's Research Hospital to prevent the progression of prediabetes to type 2 diabetes in adult survivors of childhood cancer. This intervention consists of the oral administration of metformin hydrochloride, a biguanide insulin sensitizer, alongside a digital lifestyle change program delivered via the Noom smartphone application. Metformin works primarily by inhibiting mitochondrial complex I and activating AMP-activated protein kinase (AMPK), which leads to reduced hepatic glucose production and improved peripheral insulin sensitivity. The digital component provides a core curriculum focused on diet, exercise, and behavioral strategies for goal setting and self-monitoring. The PREDM pilot study specifically addresses the high metabolic risk faced by childhood cancer survivors, who are twice as likely to develop prediabetes compared to the general population.
