Tangsu (metformin + glipizide) is a fixed-dose combination oral antidiabetic medication developed and marketed by Yangzi Jiang Pharmaceutical Group. It combines two antihyperglycemic agents with complementary mechanisms of action to improve glycemic control in patients with type 2 diabetes mellitus. Metformin hydrochloride, a biguanide, primarily works by activating AMP-activated protein kinase (AMPK), which leads to a reduction in hepatic glucose production (gluconeogenesis) and an increase in peripheral insulin sensitivity. Glipizide, a second-generation sulfonylurea, stimulates insulin secretion from pancreatic beta cells by binding to and closing ATP-sensitive potassium (KATP) channels, specifically the sulfonylurea receptor 1 (SUR1) subunit. This closure induces cell depolarization, leading to calcium influx and subsequent insulin release. The combination is typically used when diet, exercise, and monotherapy with either metformin or a sulfonylurea do not result in adequate glycemic control.
