Metformin + insulin glargine U100 + human regular insulin is a three-component combination antidiabetic regimen used to manage type 2 diabetes mellitus. It combines metformin (an oral biguanide that reduces hepatic glucose production and improves insulin sensitivity), insulin glargine U100 (a long-acting basal insulin analog targeting insulin receptors to provide steady background glucose control), and human regular insulin (a short-acting insulin for prandial glucose control). This combination was investigated by the University of Jordan in an open-label, parallel-group, randomized controlled trial examining the effect of dosage reduction of four hypoglycemic multidrug regimens on acute glycemic complications in type 2 diabetes patients who fasted during Ramadan 2017. The trial compared regular dosage versus a reduced dosage of this regimen, with primary outcomes of hypoglycemia and hyperglycemia incidence.
