Metformin + letrozole is a combination of two small-molecule drugs used primarily in the management of infertility associated with polycystic ovary syndrome (PCOS) and has also been investigated for use in recurrent estrogen receptor-positive endometrial cancer. Metformin is an oral antihyperglycemic agent that improves insulin sensitivity and reduces hepatic glucose production, while letrozole is a nonsteroidal aromatase inhibitor that suppresses estrogen synthesis by inhibiting the aromatase enzyme. The combination targets both metabolic dysfunction (via metformin) and hormonal imbalance (via letrozole), leading to improved ovulation rates, endometrial receptivity, ovarian reserve function, and regulation of LH and FSH levels in women with PCOS. In oncology settings, this combination may provide synergistic inhibition of the PI3K pathway (metformin) and estrogen receptor signaling (letrozole), contributing to tumor control when combined with other agents[2][3][4][6].
