Overview

Metformin + n-acetylcysteine + ursodeoxycholic acid is a combination therapy investigated for the treatment of non-alcoholic steatohepatitis (NASH) and non-alcoholic fatty liver disease (NAFLD). This therapeutic approach targets multiple pathways in the pathogenesis of NASH, including insulin resistance, oxidative stress, and bile acid-induced hepatotoxicity. Metformin, a biguanide, functions primarily by activating AMP-activated protein kinase (AMPK) to improve insulin sensitivity and reduce hepatic glucose production. N-acetylcysteine (NAC) serves as a precursor to glutathione, enhancing the liver's antioxidant capacity to combat oxidative stress. Ursodeoxycholic acid (UDCA) is a naturally occurring bile acid that exerts cytoprotective effects by displacing more toxic hydrophobic bile acids and modulating apoptotic pathways. Clinical studies, including a 48-week multicenter randomized trial, have evaluated the efficacy of this triple combination in improving liver histology and biochemical markers in patients with biopsy-proven NASH.
