Metformin + oligomeric procyanidin complex (OPC) is an investigational combination therapy being developed by the Medical University of South Carolina for the treatment of prostate cancer. The regimen consists of metformin, a biguanide typically used for type 2 diabetes, and OPC, a polyphenolic extract derived from grape seeds or maritime pine bark. The combination is designed to pharmacologically reduce the levels of advanced glycation endproducts (AGEs), which are known to accumulate in prostate cancer patients—particularly those undergoing androgen deprivation therapy—and promote tumor cell survival and proliferation. Preclinical studies have demonstrated that OPC induces apoptosis and inhibits growth in prostate cancer cell lines, while metformin acts as an insulin sensitizer and AMPK activator, potentially working in tandem with OPC to disrupt AGE-mediated signaling pathways.
