Metformin + pioglitazone is a fixed-dose oral combination medication used to improve glycemic control in adults with type 2 diabetes mellitus. It combines two antihyperglycemic agents with complementary mechanisms of action. Metformin is a biguanide that decreases hepatic glucose production, reduces intestinal absorption of glucose, and improves insulin sensitivity by increasing peripheral glucose uptake and utilization. Pioglitazone is a thiazolidinedione (TZD) that acts as an agonist at peroxisome proliferator-activated receptor gamma (PPARγ), enhancing insulin sensitivity in adipose tissue, muscle, and the liver.
