Metformin + pioglitazone + exenatide is a combination therapy used in the management of type 2 diabetes mellitus (T2DM). This regimen combines three agents with complementary mechanisms of action to improve glycemic control. Metformin is a biguanide that decreases hepatic glucose production and improves insulin sensitivity. Pioglitazone is a thiazolidinedione that acts as an agonist at peroxisome proliferator-activated receptor gamma (PPARγ), enhancing insulin sensitivity in adipose tissue, muscle, and the liver. Exenatide is a glucagon-like peptide-1 receptor agonist (GLP-1 RA) that increases glucose-dependent insulin secretion, suppresses inappropriate glucagon secretion, slows gastric emptying, and promotes satiety[1][6]. Clinical studies have shown that this triple therapy results in greater reductions in HbA1c with fewer hypoglycemic events compared to traditional sequential add-on therapies[1][2][8]. The combination also offers benefits such as weight loss or less weight gain compared to regimens including sulfonylureas or insulin[4][5].
