Metformin + secukinumab is a combination therapy being investigated for the treatment of moderate-to-severe plaque psoriasis, particularly in patients who are overweight or obese. Secukinumab is a fully human IgG1 monoclonal antibody that selectively binds to and neutralizes interleukin-17A (IL-17A), a key pro-inflammatory cytokine involved in the development of psoriatic plaques. Metformin, a biguanide traditionally used for type 2 diabetes, is added to this regimen to potentially enhance therapeutic efficacy by activating AMP-activated protein kinase (AMPK) and improving metabolic parameters, which are often dysregulated in obese psoriasis patients. This combination is currently being evaluated in a Phase 4 clinical trial led by Union Hospital, Tongji Medical College, Huazhong University of Science and Technology to assess its impact on clinical outcomes such as PASI75 and IGA scores compared to secukinumab monotherapy.
