Overview

The combination of methadone, dexmedetomidine, and ketamine, often referred to as the MDK regimen, is a multimodal analgesic approach being investigated by the University of Missouri-Columbia for the management of postoperative pain in patients undergoing complex spine surgeries. This combination therapy leverages the distinct pharmacological profiles of its three components to target multiple pain pathways. Methadone acts as a long-acting mu-opioid receptor agonist and an NMDA receptor antagonist; dexmedetomidine is a highly selective alpha-2 adrenergic agonist with sedative and analgesic properties; and ketamine serves as a non-competitive NMDA receptor antagonist. By utilizing these agents in tandem, the regimen aims to provide superior intraoperative and postoperative pain control while significantly reducing the requirement for supplemental opioid medications, thereby potentially minimizing opioid-related side effects and improving patient recovery outcomes.
