Overview

Methadone + morphine is a combination of two opioid analgesics used for pain management. Both drugs act primarily as full agonists at the µ-opioid receptor, producing potent analgesic effects by altering the perception and response to pain in the central nervous system. Methadone also acts as an antagonist at the N-methyl-D-aspartate (NMDA) receptor and inhibits serotonin and norepinephrine reuptake, which may contribute to its unique antinociceptive properties compared to other opioids. Preclinical studies have shown that combining methadone with morphine results in synergistic analgesia—meaning their combined effect on pain relief is greater than additive—without a corresponding increase in side effects such as gastrointestinal inhibition. This synergy is attributed mainly to activity at different opioid receptor sites, particularly involving the L-isomer of methadone[3][5][8]. The combination may be considered for severe or refractory pain when monotherapy does not provide adequate relief.
