Methadone + naloxone is a combination of two drugs used in the management and treatment of opioid use disorder. Methadone is a synthetic opioid agonist that acts primarily at the mu-opioid receptor, providing relief from withdrawal symptoms and reducing cravings by activating these receptors more slowly and with less euphoria than other opioids. Naloxone is an opioid antagonist that blocks or reverses the effects of opioids by competitively binding to opioid receptors, particularly mu-opioid receptors, without activating them. The rationale for combining methadone with naloxone would be to provide maintenance therapy for opioid dependence while deterring misuse; if taken as prescribed (e.g., orally), methadone exerts its therapeutic effect while naloxone has minimal bioavailability. If misused (e.g., injected), naloxone would precipitate withdrawal by blocking methadone’s action, thus discouraging abuse. While such combinations are well established for buprenorphine (as in buprenorphine/naloxone products like Suboxone), there is little evidence that a commercial or widely available formulation of methadone + naloxone exists as of 2025; however, both drugs are individually approved and commonly co-prescribed in clinical practice for safety reasons[1][3][6][8].
