Methotrexate + ERB-041 is a combination therapy investigated primarily for the treatment of rheumatoid arthritis, particularly in patients with active disease despite methotrexate therapy. Methotrexate is an antimetabolite and antifolate drug that inhibits dihydrofolate reductase (DHFR), leading to suppression of DNA synthesis and immune cell proliferation. It is widely used in cancer, psoriasis, and autoimmune diseases such as rheumatoid arthritis by decreasing the activity of the immune system[1]. ERB-041 is a highly selective estrogen receptor beta (ERβ) agonist. It modulates inflammation by activating estrogen receptor beta pathways, which are implicated in anti-inflammatory responses and regulation of cell proliferation[2][6][8]. In clinical studies, this combination was evaluated to determine if adding ERB-041 could improve outcomes over methotrexate alone in rheumatoid arthritis; however, while well tolerated and safe, ERB-041 did not demonstrate significant additional anti-inflammatory efficacy over placebo when combined with methotrexate[4][5]. The combination has also been explored preclinically for its potential effects on cancer chemoprevention via modulation of WNT/β-catenin signaling pathways[6].
