This combination therapy regimen, investigated by the MidAmerica Neuroscience Research Foundation at the Rowe Neurology Institute, utilizes an induction-maintenance approach for patients with clinically isolated syndrome (CIS) at high risk for multiple sclerosis. The protocol consists of a single high-dose intravenous bolus of methotrexate (2 Gm/m2) with leucovorin rescue to rapidly suppress inflammatory activity, followed by long-term maintenance with weekly intramuscular injections of interferon beta-1a (Avonex). Methotrexate is a small molecule antifolate that inhibits dihydrofolate reductase, thereby reducing the proliferation of inflammatory immune cells. Interferon beta-1a is a biologic immunomodulator that binds to type I interferon receptors to reduce the transmigration of leukocytes across the blood-brain barrier and modulate cytokine production.
