Methotrexate + procarbazine + vincristine + cytarabine (MPV-A) is a multi-agent chemotherapy regimen developed by Assistance Publique - Hôpitaux de Paris (AP-HP) for the treatment of primary central nervous system lymphoma (PCNSL), particularly in elderly patients. The regimen consists of four small molecule cytotoxic agents: methotrexate (an antimetabolite that inhibits dihydrofolate reductase), procarbazine (an alkylating agent), vincristine (a vinca alkaloid that inhibits microtubule assembly), and cytarabine (an antimetabolite that inhibits DNA polymerase). This combination was evaluated in the ANOCEF-GOELAMS randomized phase 2 trial (NCT00503594), which compared its efficacy and toxicity against a methotrexate and temozolomide regimen in patients older than 60 years. The MPV-A regimen showed favorable efficacy endpoints in this population, targeting rapidly dividing lymphoma cells within the central nervous system through multiple complementary mechanisms of action.
