Methotrexate + tripterygium wilfordii is a combination therapy used primarily for the treatment of rheumatoid arthritis (RA). Methotrexate is a conventional disease-modifying antirheumatic drug (DMARD) that inhibits dihydrofolate reductase, interfering with DNA synthesis and immune cell proliferation. Tripterygium wilfordii Hook F (TwHF), also known as thunder god vine, is a traditional Chinese medicinal plant whose extracts contain multiple bioactive compounds such as diterpenoids and triterpenoids. These compounds exert immunosuppressive and anti-inflammatory effects by inhibiting pro-inflammatory cytokines like TNF-α and IL-6. Clinical studies have shown that the combination of methotrexate with TwHF or its glycoside derivatives provides superior efficacy in controlling RA disease activity compared to methotrexate monotherapy, without significantly increasing toxicity or adverse events[1][2][5]. The combination acts synergistically to reduce joint inflammation and improve clinical outcomes in RA patients.
